Privacy Notice for Dennise Rathbone Counselling, Psychotherapy and Supervision

The processing of personal data is governed by the General Data Protection Regulation 2016/679 (the GDPR). This legislation will replace current data privacy law, giving more rights to you as an individual and more obligations to organisations and individuals holding your personal data. Examples of personal data I may hold about you include your contact details and your GP’s name and address, in the unlikely event that I may need to contact your GP in an emergency. You may request, at any time, that changes are made to your contact details.

This is how I work with any information I may hold on you; I generally complete an initial assessment form during our first appointment and make brief notes of our sessions thereafter as an aide memoir. These are stored securely and will be destroyed seven years after the conclusion of your therapy, in case, for example, you should decide to return to therapy during that time frame. I will not share this information with anyone other than my supervisor for the purposes of supervision. Confidentiality is key to the process of therapy.

In the event of my sudden illness or death, I will ensure that a professional will is in place arranging for current clients to be informed and client notes to be securely destroyed.
